    A Public Health Diabetes and Tobacco Outreach Program

    Now Accepting Youth and Adult Referrals

    The Tobacco Education Program is currently accepting referrals for both youth and adults seeking tobacco and nicotine prevention, education, and cessation support services.

    We work with youth, adults, and families to increase awareness of the health risks associated with commercial tobacco products, including vaping and nicotine use, while promoting healthy, tobacco-free lifestyles.

    In addition to cessation services, we provide community outreach and engagement activities, educational presentations, prevention campaigns, health fairs, and community events that promote wellness and tobacco-free living.

    Self-referrals are welcomed.

    We are now accepting youth referrals for support to stop and prevent use of all nicotine products, including vapes, cigarettes, and pouches.

    Our program offers confidential, supportive guidance to help youth build healthier habits and make informed choices.

    Ask your provider for a referral to the Tobacco Education Program. The referral will be sent to LCTHC Behavioral Health for Tobacco/Nicotine cessation.

    Help protect the health of our Tribal community by joining the Tobacco Community Coalition.

    Coalition members work together to:

    • Prevent youth vaping and nicotine use
    • Raise awareness about the dangers of commercial tobacco products
    • Support community education and prevention efforts
    • Promote tobacco-free environments and healthy lifestyles
    • Guide community-led tobacco prevention campaigns

    Our Vision

    To create a healthier, tobacco-free community through compassionate care, cessation services, prevention education, and community engagement. We strive to empower individuals and families to live healthier, longer lives free from commercial tobacco use.

    Our Purpose

    We are dedicated to reducing commercial tobacco use through comprehensive education, personalized support, and evidence-based interventions. We empower individuals to quit tobacco by providing tailored counseling and cessation services while actively engaging the community through outreach, education, and partnerships that promote a healthier, tobacco-free future.

    Program Background

    The Tobacco Education Program was established in July 2020 through grant funding from the California Tobacco Control Program, now known as the California Tobacco Prevention Program (CTPP) within the California Department of Public Health.

    LCTHC was awarded continued funding for the 2025–2028 grant cycle to expand tobacco prevention, education, and cessation services within our Tribal community.

    Program Goals

    Our objectives include:

    1. Implementing the Ask, Advise, Refer tobacco screening protocol within LCTHC
    2. Pediatrics to connect patients with cessation services.
    3. Developing and supporting a Tribal Community Tobacco Coalition to guide prevention campaigns and community engagement activities.
    4. Promoting commercial tobacco cessation through education, support, and treatment resources.
    5. Maintaining an active Tobacco Education Program website and social media presence to share information and resources.

    Hours: Monday–Friday, 8:30 am–5:00 pm
    Location: LCTHC Main Clinic, Lakeport
