    Our Mission

    Public Health Outreach promotes health, wellness, cultural strength, and community connection throughout Lake County Tribal communities.

    We work collaboratively with Tribal partners, local organizations, and community leaders to provide outreach, education, resources, and culturally grounded health initiatives.

    We strive to meet our people where they are—honoring tradition while advancing public health—through:

    Community Health Representatives (CHR)

    Community Health Representatives (CHRs) play a critical role in building trust, reducing barriers to care, and strengthening community wellness are certified Medical Assistants serving as vital liaisons between healthcare systems and the community. They provide culturally competent outreach, education, advocacy, and case coordination to improve health outcomes for Tribal members and families.

    Services may include:

    • Health education and promotion
    • Home visits and wellness checks
    • Assistance navigating healthcare services
    • Chronic disease education (diabetes, hypertension, etc.)
    • Medication and appointment reminders
    • Cultural support and advocacy
    • Referrals to medical, behavioral, and social services

    Transportation Services

    The Transportation Department ensures that community members have safe and reliable access to healthcare services and essential appointments.

    Services include:

    • Medical appointments (primary care, specialty care, dental, behavioral health)
    • Pharmacy visits
    • Dialysis transportation
    • Elder services transport
    • Health program referrals
    • Community event transport (as scheduled)

    Missing & Murdered Indigenous People (MMIP) Day

    The Annual Missing & Murdered Indigenous People (MMIP) Day, hosted by the Lake County Tribal Health Consortium Public Health Outreach Department, is a sacred and community-centered gathering dedicated to honoring the lives of Indigenous relatives who are missing or have been taken by violence.

    We’re proud to announce that the month of May 2026 has been declared as MMIP month in Lake County.

    This event raises awareness about the ongoing crisis disproportionately impacting Indigenous communities across the United States and Canada, while uplifting the voices of families, survivors, and advocates seeking justice, healing, and systemic change.

    MMIP Day serves not only as a memorial, but also as a call to action — bringing together Tribal communities, partner organizations, service providers, and allies to stand in solidarity.

    Collaborating with our Community–Free Events

    Big-Clean-Time Celebration

    Hosted by Lake County Tribal Health’s Recovery Support Services. A powerful celebration of the gifts of recovery. Performances by Native dancers and inspirational speakers. Recovery agencies are there to help aid one’s path to healing. Raffles and family activities. Commemorative t-shirt. Recovery certificates and coins presentation. Lunch is provided.

    A traditional gathering that brings together members of a tribe or community for cultural exchange and celebration.

    Recovery Happens

    Recovery walk and banner contest, open to all communities, agencies and individuals. Inspirational speakers who share their experience, strengths and hope. Informational resource booths, and family-friendly activities. Lunch is provided, and raffle prizes are given throughout the day.

    Veterans Stand Down

    For Lake County Veterans, presented by Vet Connect of Lake County. This event plays a critical role in helping our local veterans, their families, and active-duty personnel obtain necessary goods and services. Lunch is provided. Bus fares are waived by Lake County Transit for veterans and family members attending this event.

    Annual Kelseyville Pear Festival

    Showcasing the agricultural heritage of the Kelseyville and Big Valley areas. Main Street shopkeepers show off their products. Musicians, dancers on three stages, over 100 craft and food vendors, and special exhibits fill the town. The history of Kelseyville farming, pioneer families and special events are on display at the Pear Pavilion. The all-volunteer Festival committee expands the agricultural exhibits, the Tractor and Engine Show, fine art shows, and areas for children.

    Dia de los Muertos Festival
    Festival de Dia de los Muertos

    Hosted by Lake County Tribal Health and Latinos united of Lake County, this fun family event features live music, traditional dances, and a Catrina/Catrín costume contest. Fun activities for all ages, delicious food vendors, and informational booths. Includes a Halloween costume donation drive, so bring your gently used costumes to support local families. Proceeds from the event go to college scholarships to local LatinX students.

    Health & Wellness Expo
