Diabetes Lifestyle Coach created an easy exercise routine for diabetics. All you need is a couple of dumbell weights (please check in with your consultant for weight recommendation), a theraband, and a comfortable floor or exercise mat.
HomeMeds is an evidence-based intervention that identifies potential medication-related hazards for older adults.
This FREE assessment of prescription and over-the-counter medications will be conducted by a trained Lake County Tribal Health staff with you over the phone.
If there are any potential problems, a pharmacist will review the medication list and work with your doctor to ensure there are no medication-related hazards.

Walk With Ease (WWE) is a 6-week long evidence based program that helps people with arthritis or other related conditions.
You will learn how to reduce pain, increase balance, increase strength, improve walking pace, and improve overall health.

The Lake County Tribal Health Consortium (LCTHC) Tobacco Education Program (TEP) is a public health initiative dedicated to preventing commercial tobacco use and supporting individuals who want to quit. Our program provides education, prevention services, free cessation support, community outreach and engagement, advocacy, and awareness activities to help create a healthier Tribal community.
Whether you are interested in quitting smoking, chewing tobacco, or vaping nicotine products, our team is here to support you every step of the way. Cessation services are provided at no cost and are available in person, over the phone, or virtually through Zoom, making it easier to access support wherever you are.

Currently, 1 in 3 American adults have prediabetes, and more than 80% of them don’t even know they have it. Without taking action, many people with prediabetes could develop type 2 diabetes within 5 years. With numbers like that, it’s important to learn about prediabetes and take action.

provides interactive home-based visits with a family Health Coach to improve

Project Sunset is a FREE Tobacco Youth Cessation Workshop presented by Lake County Tribal Health’s Tobacco Education Program (TEP) for all Native youth who smoke, vape, or chew commercial tobacco, or those who want to come learn more about what “cessation” means.

The Hinth’el Wellness Council is a community advisory group that works with tribal leadership to empower and promote wellness in our community to educate and restore healthy minds, bodies, and spirits by preventing diabetes and managing its complication among our Native families and diabetic patients of Tribal health.

Our Garden Program offers a welcoming and community-centered space that combines hands-on gardening with wellness education. Through our garden classes, participants explore a variety of topics, including sustainable growing practices, healthy living, and cultural connections to food and land.
Each year, the garden produces approximately 5,000 pounds of fresh food, which is shared with the local Native community. In addition to organized classes, we encourage Native patients and elders to visit the garden during the growing season (Tuesday through Friday) to harvest their own fresh produce, supporting both nutrition and a deeper connection to the garden.
All garden classes are open to Native community members as well as patients of Tribal Health, creating an inclusive environment that promotes learning, wellness, and community engagement.
