    Health Education

    FOR NATIVE PATIENTS ONLY

    Diabetes Self Management Workshop (DSME)

    • FREE Workshop designed to help you learn skills to better manage your diabetes day to day
    • You will learn tools for successful diabetes self-management, problem-solving, and action planning.
    • Will meet with a Registered Dietitian Nutritionist before and after the workshop
    • Will receive FREE program materials
    • 7-Weekly in-person sessions led by a trained facilitator from Lake County Tribal Health
    • Incentives and raffles of wellness products

    Chronic Disease Self-Management Workshop (CDSME)

    FREE workshop supports self-management of ongoing health conditions such as arthritis, heart disease, diabetes, chronic pain, high blood pressure, obesity, lung disease, and cancer.

    • Sessions are 1.5 hours once a week for 10 weeks
    • Participants will benefit from exercise, socializing, and Bingo prizes

    BingoCize

    Bingocize® is a health promotion program that combines bingo with easy exercises. Come play bingo while participating in simple exercises that promote balance, flexibility, and techniques to reduce falls.

    Elders and family members are welcome! Registered participants can expect to:

    • Play Bingocize
    • Enjoy food
    • Socialize and meet new people
    • Exercise
    • Win prizes
    • Have fun!

    Diabetic Exercises

    Diabetes Lifestyle Coach created an easy exercise routine for diabetics. All you need is a couple of dumbell weights (please check in with your consultant for weight recommendation), a theraband, and a comfortable floor or exercise mat.

    HomeMeds

    HomeMeds is an evidence-based intervention that identifies potential medication-related hazards for older adults.

    This FREE assessment of prescription and over-the-counter medications will be conducted by a trained Lake County Tribal Health staff with you over the phone.

    If there are any potential problems, a pharmacist will review the medication list and work with your doctor to ensure there are no medication-related hazards.

    *Only available to those who have participated in Diabetes Self-Management Workshop or Chronic Disease Self-Management Workshop

    Walk With Ease

    Walk With Ease (WWE) is a 6-week long evidence based program that helps people with arthritis or other related conditions.

    You will learn how to reduce pain, increase balance, increase strength, improve walking pace, and improve overall health.

    Registered participants will receive a guidebook and pedometer.

    Produce Prescription–Food for Healing

    Healing begins with what we eat. Nourish your body. Honor your culture. Reclaim your wellness.

    Food for Healing is a produce prescription program that offers free fresh fruits and vegetables to Native families in Lake County. Through culturally-rooted wellness, we use food as medicine to:

    • Support diabetes and chronic disease management
    • Learn how to incorporate fruits and vegetables into your meals
    • Reconnect with ancestral food traditions
    • Create delicious fresh produce dishes

    Participant benefits:

    • FREE produce boxes twice a month
    • Monthly check-in with Food Farmacist
    • Free recipes, tastings, and samplings
    • Work toward improving A1c, blood pressure, lipids, and weight

    Tobacco Education Program

    We’re here to help!

    The Lake County Tribal Health Consortium (LCTHC) Tobacco Education Program (TEP) is a public health initiative dedicated to preventing commercial tobacco use and supporting individuals who want to quit. Our program provides education, prevention services, free cessation support, community outreach and engagement, advocacy, and awareness activities to help create a healthier Tribal community.

    Whether you are interested in quitting smoking, chewing tobacco, or vaping nicotine products, our team is here to support you every step of the way. Cessation services are provided at no cost and are available in person, over the phone, or virtually through Zoom, making it easier to access support wherever you are.

    Who is at Risk for Prediabetes and Diabetes?

    Currently, 1 in 3 American adults have prediabetes, and more than 80% of them don’t even know they have it. Without taking action, many people with prediabetes could develop type 2 diabetes within 5 years. With numbers like that, it’s important to learn about prediabetes and take action.

    Take the CDC online test to find out if you are at risk for prediabetes and type 2 diabetes.

    Youth Programs

    Together We Can

    Together We Can is a program for Native American Youth at risk for diabetes. It
    provides interactive home-based visits with a family Health Coach to improve
    education and social support for youth and their families.

    Project Sunset

    Stop Using Nicotine. Start Engaging Traditions.

    Project Sunset is a FREE Tobacco Youth Cessation Workshop presented by Lake County Tribal Health’s Tobacco Education Program (TEP) for all Native youth who smoke, vape, or chew commercial tobacco, or those who want to come learn more about what “cessation” means.

    Hinth’el Wellness Council

    The Hinth’el Wellness Council is a community advisory group that works with tribal leadership to empower and promote wellness in our community to educate and restore healthy minds, bodies, and spirits by preventing diabetes and managing its complication among our Native families and diabetic patients of Tribal health.

    Our Mission and Values:

    • Empower the community by promoting Native Wellness through community events and LCTHC Diabetes and Tobacco programs.
    • Strengthen community-clinic linkages by sharing Native perspectives and vision for Native wellness, and advocating for the community.
    • Lead strategic planning and program implementation by working directly with program staff to improve programs for our Native community.

    Garden Program

    FOR NATIVE PATIENTS ONLY

    Our Garden Program offers a welcoming and community-centered space that combines hands-on gardening with wellness education. Through our garden classes, participants explore a variety of topics, including sustainable growing practices, healthy living, and cultural connections to food and land.

    Each year, the garden produces approximately 5,000 pounds of fresh food, which is shared with the local Native community. In addition to organized classes, we encourage Native patients and elders to visit the garden during the growing season (Tuesday through Friday) to harvest their own fresh produce, supporting both nutrition and a deeper connection to the garden.

    All garden classes are open to Native community members as well as patients of Tribal Health, creating an inclusive environment that promotes learning, wellness, and community engagement.
