Please note: The age-out automation for minors is set at 18 years old. This means if a parent or guardian has access to a Patient's Portal, the system will automatically deny their access upon the patient's 18th birthday. If the patient would like to grant access to their parent or guardian again, they may do so by filling out a form at any of our medical front desks.
